Overview
Basal-bolus therapy is the gold standard for intensive insulin management, providing both fasting and prandial glucose control through separate basal and bolus insulin components.
Regimen Components
Basal Component
- Purpose: Suppress hepatic glucose output between meals
- Options: Glargine, degludec, detemir (once or twice daily)
Bolus Component
- Purpose: Cover postprandial glucose excursions
- Options: Lispro, aspart, glulisine (before each meal)
Dosing Guidelines
- Basal: Start 0.2-0.4 U/kg/day, titrate by 2 units every 3 days
- Bolus: Start 0.1 U/kg per meal, adjust based on carb ratio
- Total daily dose: 0.5-1.0 U/kg/day
